Peptides explained: what they are, what they do, and the real risks

Peptides range from life-saving drugs like insulin to unregulated gray-market injections. We review the evidence from NIH, FDA, Harvard, and the Cleveland Clinic.

Peptides are suddenly everywhere: in weight-loss clinics, on wellness influencer feeds, and in vials sold “for research purposes only.” But what actually are they, what do they do in the body, and is the hype safe? We dug into the peer-reviewed literature and the positions of the FDA, Harvard Medical School, and the Cleveland Clinic to separate the proven from the promotional.

What peptides are

A peptide is a short chain of amino acids, the building blocks of protein, linked by peptide bonds. NIH’s StatPearls reference defines them as roughly 2 to 50 amino acids; longer chains are polypeptides or proteins. Your body already makes countless peptides, each with a specific job: insulin for blood sugar, endorphins for pain and stress, and growth hormone-releasing hormone, which tells the pituitary to release growth hormone. Lab-made versions are the active ingredients in many approved medicines.

What they do: the proven side

Some peptide drugs are among the most validated in modern medicine. The clearest are insulin and the GLP-1 class for obesity and type 2 diabetes, with liraglutide (2014), semaglutide (2021), and tirzepatide (2023) approved for chronic weight management. A 2022 Nature Reviews paper counts hundreds of peptide-based drugs in oncology, diabetes, cardiovascular, and antimicrobial use. In skincare, the Cleveland Clinic notes certain signal peptides can stimulate collagen, though results are gentler than procedures.

The social media buzz is not about insulin. It is about the gray-market injectables in the lower rows, sold “for research purposes only.” Harvard notes these are not regulated or approved by the FDA.

The gray market

The regulatory picture is stark. The FDA placed BPC-157 and TB-500 on its Category 2 list of substances that may present significant safety risks, citing immunogenicity, impurities, and missing human data. A July 2026 FDA advisory committee heard staff scientists say there was little data these could be used safely, with no human TB-500 studies at all.

Potential positive effects

For approved drugs, benefits are well documented: blood sugar control, sustained weight loss, and reduced cardiovascular risk and better sleep apnea with GLP-1s. For the experimental peptides, signals are preclinical only. Harvard acknowledges animal studies hint BPC-157, CJC-1295, and GHK-Cu might aid healing, but large human trials are still needed.

Potential negative effects and unknowns

The safety gap is the biggest concern. Dr. Pieter Cohen of Harvard says unregulated injectables amount to “experimenting with injecting amino acids into your body,” and some may trigger immune reactions including anaphylaxis. The FDA separately warns gray-market vials may be contaminated. Reported side effects are mostly anecdotal: nausea, diarrhea, headache, fatigue, injection-site irritation. The AMA adds a possible pancreatitis risk and that much remains unknown.

The takeaway

Peptides are not a scam category. As approved drugs, insulin and GLP-1s are among the most important medicines of our era. The problem is the leap from those to unregulated injectables bought online. The proven benefits belong to specific, FDA-reviewed molecules studied in tens of thousands of people. The trendy “research” peptides have almost no human data, a known contamination risk, and an explicit FDA safety flag.
